The worker-access reference still said placement read preparation joins an

overlapping publication's settlement and rereads fresh facts. #168747 removed

that loop and accepted the tradeoff that an ordinary placement read racing a

same-session mutation reports authority changed. Describe the current

fail-closed contract and why rereading only the preparation read would not

remove the window.
