Retain mock Responses exceptions with the request scenario, advertised tools,
and input identities so a silent reporter does not hide an incomplete stream's
cause. On missing revisions, include selected pins, Gateway revision inventory,
publication and placement receipts, node logs, and observed bundle hashes.
Expand catalog paths against the actual Gateway or worker fixture home.
This is diagnostic-only: all 75 assertions, deadlines, and runtime behavior are
unchanged. No worker resource path is inferred when the turn never reaches the
provider.
Proof: controlled incomplete-stream and missing-revision failures exposed the
expected context through the default silent reporter. Clean Node worker E2E
passed (46.069s body, 72.94s total), targeted E2E types, lint, format, and P2
review passed. Runtime proof used main 095f94c; the diagnostic files remained
unchanged when integrating newer main. Final offline install refresh could not
complete because registry metadata for @agentclientprotocol/codex-acp was absent;
the original frozen offline install and prepared runtime had already passed.
